The most likely diagnosis is adenomyosis - a variant of endometriosis
which involves the muscular wall of the uterus. Second in line would
be adhesions or scar tissue. There also could be persistent endo but
this is the least likely issue at your age. Solutions to the problem
would include hormonal suppression or surgically removal of the
adhesions and possibly even hysterectomy if the problem was severe
enough and you were unable to be successful with medical therapies.
