PCOS: nonfunctional ovarian cysts

i am a 41 y/o woman. i have had pcos for 3-4 years, that i know of. at the moment i have a nonfunctional, 4 cm, mildly complicated, right cyst. over the past 6-9 mos., i have menstruated every 3 mos., and when i do, it is much more painful than it used to be, but only on right side. i often times suffer from severe rlq pain even when i'm not menstruating. (my older sister is 2 years older and continues to menstrate regularly, every 3 weeks for her.) my mother had a total hysterectomy in her 30's due to a fibrous uterus and died at 54 t/o of metastatic breast ca. my paternal grandmother had bone ca when she died, i believe she was in her late 70's. i am 0 grata, morbidly obese with a family history of adult onset dm (my father also has cad and renal disease due to dm), have metabolic syndrome and smoke 1/2 ppd.

my ob/gyn is taking a wait and see attitude, in part due to the increased risk of heart disease after an oophorectomy. i can understand this approach, but do not agree with it, as i have developed several cysts through the years. even if this current cyst dissipates on its own, i will only develop more cysts. as i have begun doing more research on pcos, as i understand it, having nonfunctional cysts give me an increased risk of ca, something i already have due to both my family hx and being a smoker. to further complicate matters, my husband and i are planning a move to tombstone, az. we currently live ~1 hr from boston, ma, where there are some of the best doctors and hospitals in the world. my current ob/gyn is in a smaller town. i have begun looking into a 2nd opinion in boston. however in the meantime, i was hoping to get a medical opinion on line.
