This is called a Doderlein hysterectomy. An incision is first made in the posterior fornix and the uterine fundus is pulled through the opening and the hysterectomy is performed "top down" as it would be for an abdominal hysterectomy. There is a limitation to the size of the uterus that can be removed this way.
