Generally, you need an order from a doctor for any testing, and personal details needed are usually those for billing, etc.--not clinical things like why you're having the test done.

There are free standing labs in my area (metro Atlanta, Georgia) called "Any Lab Test" and somehow it looks like you can walk up and do anything without an order. Sorry, but I have no idea how that works.
