YEARLYEXAM: No Breast Exam with Yearly Pap?

Hi, I recently went in for my yearly pelvic exam and Pap smear. While there the Dr. did not perform a breast exam. I have a 6 month old and I also saw this same Dr. throughout my pregnancy. He was fabulous, but he never once examined my breasts then either. When I went in for my pregnancy appts., I was provided a sheet for the bottom half only. This time he told me where they were in a drawer. I noticed some for the top as well, but I only undressed from the bottom half, as I wasn't sure how things would be done (never had a yearly from this Dr. before). I am wondering if it is customary to not do a breast exam with Pap due to my age? (I am 28) Or if it was because I did not undress on top and he could not legally ask me to? Would it be construed as me refusing the breast exam? I'm just wondering what other reason he would have for not doing one and if this is normal. I'm pretty sure my previous female doc would have just asked me to use to sheet on top too if I had misunderstood her. Should I have had a breast exam? Thanks!
