You raise some GREAT questions about diaphragm. I'll see if I can answer them. 1. You can get a diaphragm fitted by an Ob/gyn, nurse practitioner, or physician's assistant. This is a basic skill, learned by everyone who does woman's health care. A Family planning clinic through a health department of planned parenthood can fit you quite easily. Yes, it is fitted vaginally, similar to a pelvic exam, when they examine your uterus and ovaries. I highly recommend an exam to get the best fit. 2. There are other options similar to the diaphragm, so it might be helpful to discuss these with your gyn provider, and see if you can find a good method that would meet your needs. Another possibility is a progestin only pill, such as Micronor. This would decrease the estrogen effects, while still providing you with effective contraception. 3. Effectiveness of the diaphragm isn't as high as with the birth control pill. Consistent use, and using with a modified natural family planning method (identifying your ovulation signs, and actual ovulation) will help improve this effectiveness. 4. Some couples do feel the diaphragm with intercourse. Some are just more sensitive to the object in the vagina. But, fitting with the correct size helps this to be minimal. Think of it as a raincoat over the round cervix. 5. It might or might not aggravate the yeast infections. If you get them monthly, I would be suspicious that you are eating more sugars around that time, and this sets you up for a yeast infection. So, track your eating and see if you can determine an association between the yeast infection and your diet. Many women notice more itching around their period, and usually find they are craving more sugar at this time. It's possible the yeast infections are associated with the hormonal changes that occur as you go through your cycle. So, eliminating them is a wise direction.
