I would recommend that a hysteroscopy and D&C be done if it has not been done within the past year because there is no way to know if this is from fibroids, hormones, or something ore ominous. If the workup is complete and there are not serious findings then it is OK to just observe until yet another year has passed when another look should be considered. An endocervical curettage and endometrial biopsies are a good idea.
