I would recommend a cooperating urogynecologist and colorectal surgeon as well as some connective tissue disease testing for diseases of connective tissue that cause weakness of support tissue.

Future deliveries should be by cesarean section. A gastro enterologist should be involved in correcting the underlying constipation and straining or the condition will recur again.

Sounds like there might be a sliding enterocele between the vagina and the rectum, a loop of small bowel that gets in the rectovaginal septum and needs to be corrected as well.
