GYNECOLOGIC: Chronic yeast infections

For the last 7 months I have been getting monthly (at least) yeast infections. I have always been prone to get them but I am having at least one per menstrual cycle now. I recently went to my doc about it and received a Rx for Diflucan. It worked on that infection but now I have another (2 weeks later - though I just took an antibiotic and had back surgery so that may be causing this most recent one).

I'm wondering what is causing this? I am sexually active, but have not been on BC pills for almost a year. We use condoms so we have experimented - there's no difference when we use non-lubricated or non-spermicide condoms. There's also no difference if we simply don't use condoms at all. I've been following an ultra low-carb diet for 5 months to drop weight. I am on pain medication (oxycodone). Was on 8 - 12 pills per day now taking 3 / 4 per day. Hope that's enough information!
