33 yr old white female with history of endometriosis and thin adhesions on left tube, ovary, bowel and peritineum wall. Lap done Oct 06, adhesions taken down. Endo not visualized at that time. No pain since lap in Oct 06. On continuous Loestrin and prometrium days 12 -21 of loestrin pill pack. Have been on different birth control pills before Loestrin was tried. Had a ultrasound in Jan 06. The ultrasound showed ovulation while on the pill (different one than the loestrin). I did break through with bleeding in March because I forgot one pill. Had big clots followed by med to light bleeding for 4 days. Broke through and had spotting in April. Only one day. Some pain, 400 mg motrin took care of it easily. Last wednesday (5/16), severe sharp abdominal pain on the lower right pelvic and lower right back that came on all of a sudden. Followed an hour later by a small gush of blood (vaginal bleeding). Followed by continued lower right pelvic and back pain, nausea, spotting to light vaginal bleeding from Wednesday night to now (Tuesday). Bleeding stopped Monday night. I was seen in the ER (Monday Night)to rule out appendicitis (couldn't get in with obgyn or rather the nurse would not make me an appt) so my pcp saw me and sent me to the ER. Appendicitis was ruled out. The er drs said it sounds like an ovarian cyst ruptured or the endo/adhesions are back. The obgyn nurse (same one that would not give me an appt) was questioning 3 drs diagnosis (2 er docs and 1 family practice). Before I went to the ER, my pcp said in order it was probably appendictis, ruptured ovarian cyst, or the endo/adhesions are back. The nurse did give me an appt for Friday. Questions: 1) The ER drs said cat scan was normal but said ovarian cysts do not always show up. Is this true? Nurse made it sound like the cat scan shows everything and questioned the ruptured cyst diagnosis. The er dr said that my ob should be doing an US when I see him. They didn't do an US in the ER. 2) Will an US show an ovarian cyst? What about one that ruptured a week ago? Can it see the scarring/evidence left behind in a rupture? 3) Does this sound like a ruptured ovarian cyst or endo/adhesions? 4) If it is a ruptured ovarian cyst (obviously ok as not hemmoraging, just bad pain), will this cause scarring and possible more adhesions on the right side/ Thank You for your help. I would like to know more before I see the dr.
