INFERTILITY: Role of laparoscopic treatment of endometriosis in patients with failed in vitro

I concieved normally first time & had intrauterine death at 20 weeks, following which I could not concieve again for a year, subsequently I had got tests done for fertility ,all test were normal including tubal patency test,but as they found dense adhesions on laparoscopy , they did open adhesiolysis with correction of my retroverted uterus,I was also given clomiphene for a month, they also found a patch of endometriosis at that time which was cauterised. still after 6 months I did not concieve , so they did a scan which showed bilateral endometriomas, again I had a laparotomy adhesiolysis & (rt) ovarian cystectomy , drainage of (Lt) broad ligament endometrioma, they also found bilateral tubal blockage in lap dye test. my menstrual periods are 27 -28 days cycle , I had no symptoms of endometriosis whatsoever.

I had IVF 5 months after the surgery which failed, so I contacted another specialist who has now suggested laparoscopic adhesiolysis , with removal of endometriotic plaques & release blocks from tubes ( fimbrial end ) & checking for cervical incompetence as well stating that this would

1) improve the chances of me concieving normally

2) Improve my chances of getting pregnant with IVF

I am skeptical & confused ,what would be your advise regarding the surgery & what would be the sucess rate if I undergo IVF with & without the above surgery. I would also like to know the chances of me having ectopic pregnancy if the tubes were unblocked & I concieve normally.
