Yes! Got go to the record dept at the facility where you had the lap done.[16 days is plenty of time for them to have it, if they don't threaten with an attorney!] Sign the release paper and tell them you will wait while they print up the operative report. They did it for me here in sac ca. Hopefully they will you too! They can't deny you the report. All the dr office has to do is get it from your chart while you are there and run off a copy.,. tell them you need it by a certain date....like yesterday! If you were given any drug without your explicit consent, there is a problem. Lupron issues aren't something that should be taken lightly. Go to one or the other and stay there until they give in. This is YOUR info and you are entitled to it. BEst of luck. Joyce
