Irregular periods - Doctors, what next?

I'm 39 years old, menstruating since age 12; not on the BCP. For the vast majority of my life, my periods were like clockwork - cycles of 28 or 29 days, of predictable duration and flow. However, for the past seven months, my periods have become quite irregular in all respects. January - 30 days and extremely heavy flow that necessitated a trip to the emergency room. February - 28 days and in all respects normal for me. March - 44 days and lighter than normal flow. April - 31 days and normal. My next period did not arrive until mid-June (46 day-cycle), and it was very heavy, particularly during the seventh and final day. Now my latest period has arrived, after 50 days, and it is again rather heavy.

In January, a pelvic ultrasound and CT scan revealed a huge left ovarian dermoid with a normal right ovary. There was also a 3 cm. fibroid located on the outside surface of the uterus. Later in January, I had an endometrial biopsy that was normal.

In February, I underwent open laparotomy to remove the left ovary and the dermoid. My surgeon noted that the fibroid had not increased in size, and the right ovary appeared normal. Recovery was uneventful.

In March and again in June, I had my thyroid and hormones tested. Everything was normal.

It is clear that something is off here, and I'm scheduled to see my OB-GYN again in late August. In the meantime, can you give me some insight into what I should be considering as possible issues? The fibroid? PCOS or other ovarian cause? Time for another ultrasound?
