Renee: though I agree with most of what you say, I'm a little confused by your mention that doctors get paid for allowing drug reps to sit in with a patient. I've worked in a number of medical offices and have never seen this occur. I'm also having difficulty imagining what the fee would be for -- if the drug rep is not promoting drugs while in the room, what is he/she doing there that is worth paying money to the doctor for? Please enlighten me. Thanks. >

The doctors get a GENEROUS FEE each time a drug rep sits in with a patient, so they are likely to present it to the patient in such a way to make her less likely to refuse. This is highly unethical! Also, remember that the patient is not usually feeling well and just wants to get service that day without making a fuss. The doctor and drug rep can count on that fact for compliance.
