I haven't given birth, but I did get a nasty first degree tear from having non lubricated sex with my um...large..boyfriend. At the time I didn't think to go to a doctor and get stitches, which I needed. I just waited for it to heal. Sex was extremely painful for about six months, even though the tear looked like it healed and my regular doctor said that it looked fine. I thought I was having symptoms of vulvodynia so when I went to get my dreaded annual pap smear as asked my doctor about the condition. He took a closer look and he said I have a very slight scar from the tear. Scar tissue doesn't strech like the rest of the skin in the area so the skin around the scar streches more to compensate, causing pain and more tearing. He told me to use KY silk-e on the area whenever I have sex. He also said to use some silk-e on the area once a day and stretch the skin a little bit so that it loosens up. Hope the info helps.
