When a pelvic exam is done two things are done: 1. an exam using a speculum which is shaped like the bill of a duck (when this is inside they can visually check the cervix and the vagina for problems) and 2. a bimanual exam, this is where one hand is inside the vagina and the dr is checking the shape of the uterus, ovaries, and fallopian tubes to make sure the shape is normal but they are also feeling for lumps and bumps for example that can affect the texture.

If you hurt during any part of the exam let your dr know. Pain and tenderness is an indicator that something could be wrong.

I hope that this helps.
