Spotting before periods for 3 years now!

I have been spotting about 4-6 days prior to the start of my period for at least 3 years now, and have been to two different family doctors that didn't think I needed to see an ob/gyn! I have had an ultrasound (not vaginal) which appeared normal, pap smears which are within normal limits (but sometimes partially obscured by inflammatory exudate), my thyroid checked and is normal, and I had the hormone tests done, but at the wrong time (my dr. didn't tell me I should have went on my 21 day of cycle so I got blood taken on the 11 day, so if it's luteal phase defect, one wouldn't be able to tell, am I right?)

Other symptoms I have are often fatigued, chronic low back pain (although I do a lot of sitting at a computer and lack of exercise), sometimes I get migraine headache just before my period, or during, sometimes bleed after intercourse, but not every time, and I've been diagnosed with IBS in the past, but don't often have those symptoms anymore. Also, my sister has endometriosis and she almost died from an ectopic pregnancy! My family dr. wants me to wait until we try to conceive and after I have a miscarriage before sending me to an ob/gyn!

Is there a way to tell whether this is luteal phase defect, or endometriosis, other than a laparascopy? Or could this even be something else? Also, I don't have severe cramps, just slight cramping, and I'd say my menstrual flow is average and not overly heavy, but I've read that some women don't have any of the typical symptoms of endometriosis. My sister also has hypothyroidism. Is this common in people that also have endometriosis? Thanks for any help!
