Vestibulitis Treatment

I was recently diagnosed with vestibulitis (which is incredibly relieving to know after having an unknown problem for 2.5 years) My doctor is starting out having me apply topical steroids (corticosteroids - topicort) to the area around the Bartholins glands, in the lower part of the vestibule. However, I do have irritation throughout the vestibule, even up closer to the urethra but the lower half is more sensitive to Q-tip pressure.

Would it be wiser to apply the steroids to the whole vestibule area where irritation is experienced or will it be effective to just use it on the lower half?
