Odd pain from UTI?

I recently got a VERY bad UTI which was extremely painful during urination AND when not urinating--pain in the opening of the urethra even when not urinating. This pain persisted even with Uristat doses. I began taking Macrobid, but it's been 2 full days and I still have the pain (although less of it). In my past UTI's, the pain has gone away much earlier in the antibiotic treatment and I haven't had the accompanying pain in the urethra, so I am worried. How long does it typically take for antibiotic to really kick in?

In addition, I have a sharp internal pain just below my belly button that comes sporadically. Is this a normal sympton of UTI's, or does all of this add up to something more serious?
