Not a doc but...you might want to post this on the endo forum also. There are no docs there but a lot of women that are struggling with endo. Yes, endometriosis can come back. If it is endo that is causing this (and it sounds like a good possibility from your symptoms)the ultrasound most likely will not show anything. Unfortunately the only way to tell if it is the endo is a lap. Make sure that the surgeon is skilled and experienced in dealing with endo. Unfortunately a hysterectomy does not guarantee that endo will not recur. If you end up going that route you will probable want to seriously want to consider having the ovaries removed and refraining from estrogen replacement for 3 - 6 months to let any remaining endo to "die off".
