Help!! 3rd/4th tear confusedFrom: Alan (anonymous@obgyn.net)Mon, 17 Feb 2003 03:40:07 -0600 (CST)
My parnter had a child 3 years ago and recieved a third degree tear which was missed by the doctor or he failed to inform (we did not know until after the second op) , she as since under gone one operation to repair anal and vagina- anterior sphincteroplasty/ overlapping anal repair and perinorhaphy they could not could differentiate internal from scarring external sphincter. Since this she as become faecal incontinence-Signed consent for only the perinorhapy but not overlapping repair. Delivery- delivery with Keilland forceps which were placed using the Wandering Method and rotated from LOT to ROA? Is this normal?I thought OA was the position to aim for, during the forcep delivery the blades slipped and to top it off the infant when delivered in the ROA position the blades were over her face and back of the head? The infant had slight brachial plexus injury and could move her left arm very little for 4-5 weeks but fully recovered now. My parnter was given 3x iv antibiotics and lactulose(stool softner)after the delivery and the theatre note state 2nd tear, but the labour summary notes have been changed from a 3rd to 2nd. Any ideas?any help?I am confused is this normal? Has anybody had an experance like this? Thanks
