check firswt to exclude fibroid or endometrial hyperplasia which will need endometrial biopsy..prolonged duration without a period, indicate you are not ovulating with continuous estrogen secretion unopposed with progesterone which increases the chances of endometrial hyperplasia.after excluding any lesion you can start progesterone to regulate the cycle of use fertility drugs if you want pregnancy
