Kidney Infection with out a UTI first?

I am 20 and currently on ortho tri-cyclen. I have normal paps and no STD's. I woke up in the middle of the night on Saturday night with an excruciating pain in my lower back. I also felt feverish so I took my temperature. I had a 103.7 temperature. When I urinatred there was blood and some clots in the toilet. The next morning I had my boyfriend take me to the ER. A urine a blood test revealed a kidney infection and a pretty bad bladder infection. They gave me Cipro (500mg for 3 days) and the doctor told me next time to seek tratment for a UTI before it becomes kidney infection, which can be lethal if untreated. As far as I know I had no symptoms of a bladder infection. On Friday I realized that I had to pee alot, but I was drinking alot of water that day. I had no pain or anything.

Can you get a kidney infection without a UTI first. Can a kidney infection migrate to the bladder? I thought it was the other way around. Also isn't the standard tratment of a UTI with Cipro 500mg for 7 days not 3? If the antibiotics don't work they told me to make an appointment with a urologist. I'm kinda scared and confused about this whole thing. Please Help....Thanks
