Repair of perineal tear

Two part question:
When would be the time for the repair of perineal repair to take place.
I am about two months pregnant right now. What type of doctor would
perform the surgery and why is it important not to close it too much?
Second part is about perineal excercise. I have tried after my first
birth and it has not been particularly effective. Are there any
suggestions here in terms of the types and procedures of exercise?
