I'm not a doctor. However, I experienced something similar many years ago. The pain was severe, constant and it seemed to "tingle", kind of like the picture you used to see of a radio tower and circles of "energey" radiating out from it. My doctor actually had to use SEVERAL local injections to numb the area before he could do a rectal and even then I nearly hit the ceiling with the increased agony of the exam. He sent me directly to a colorectal surgeon the same day and I was scheduled for and had surgery before I had time to realize what was going on. It was a badly infected rectal crypt or crypts and they did a rectal cryptectomy. Haven't had any problems since and that was 20 + years ago. Don't know if this is similar to what you are experiencing or that it has ANY relation to what is going on with you.
