I'm sorry. It didn't occur to me that you might not have insurance. On your ER visits have you told the treating doctor in ER that you have no insurance? They might then order more extensive Dx'ing tests than they normally would in an ER setting. Often they can refer you to medical services for those with no insurance. At the very least they might discuss with you any possible causes. If you are near a large teaching school such as a university medical school you should be able to get medical treatment beyond ER services there without insurance. Also, repost your question as new to catch the doctor's eyes here.
