If by "pouch", you mean a fold of skin over the incision, it happens frequently and is somewhat related to incision site selection. There is a natural little crease there that just begs to have the incision made right there. If it is, then there usually is a little "pouch" afterwards. It also may be related to the manner in which the skin is closed.
