Melissa, IUDs seem to work mostly by preventing sperm from fertilizing the egg. With copper ones (like Paragard), there is a sterile inflammation in the uterus which inhibits sperm function. With hormonal ones (like Mirena), the cervical mucous becomes very thick and makes it difficult for sperm to pass through. There are other effects that may add to effectiveness, like unfavorable uterine lining for implantation, but it seems the main way IUDs work is preventing conception. I am not a doctor, but since one hasn't answered you yet, I thought I'd help out.
