60 y o, postmenopause 10 years. History: rectal crypectomy 1971; tubal ligation, bladder, uterine suspension, appendectomy 1973; Crohn's disease small intestine Dx'd 1975, resection 1978, 20 year remission, flare 1998, barium series revealed partial obstruction at resection, terminal ileum deformed, minimal obstruction, probably 5-6 inches involved w/regional enteritis. Surgery recommended; declined. Alternating antibiotics monthly for bacterial overgrowth, 16 Pentasa daily accomplished remission.

30 days ago bloating started, traveling pain upper & lower left quadrant, burning pain naval area. For first time since Crohn's Dx minor pangs in lower right quadrant. Abdominal CT scan revealed normal appearance abdomen & pelvis except for 5cm x 4cm cyst left ovary. Abnormality of intestinal tract not seen.

Abdominal, endovaginal ultrasound showed 6.1 x 2.2 x 3.4cm uterus, minimal free fluid endometrial cavity, endometrial stripe normal at 4mm. Left ovary enlarged 6.0 x 4.4 x 4.9 cm w/cyst measuring 5.1 x 4.14 x 4.3 cm w/in left ovary, good true transmission and sharp backwall. Neoplastic ovarian cyst cannot be excluded.

Tetracycline 500 mg AM & PM for bacterial overgrowth completed. Resolution of pains. Bloating remains. 1/2 way thru dinner last night, w/no warning, no nausea, vomiting unrecognizable contents, nothing obvious from meal itself. Sweating. Chills.

Ceretec WBC scan scheduled next week for Crohn's to determine if any intestinal inflammation. First appt ever w/a Gyn not due until late August. No knowlege of this Gyn's experience/capability. If Crohn's inflammation found Entocort suggested.

If WBC scan negative for inflammation in intestine is it wise to wait until late August for Gyn exam & appt??? Is Entocort safe to take w/ovarian cyst?

Mayo Clinic, Cleveland Clinic, U of MI Med Center, U of Indiana School of Medicine, U of Chicago Hospitals all w/in easy access.
