I would never second guess one who has done an exam but I think your doctor is giving you, as we say, worse case scenario, so that you have, what the lawyers call, informed consent.

Varies greatly.

You may wish a second opinion on the surgery to glean more info if you are uncomfortable, In residency, you do thousands of surgeries so I'm not too concerned about someone 5 years out.
