How's that? If you drink, you will die. How's that? Your smoking will interfere with the gas exchange of the anesthetic, and your drinking will increase your risk for inhalation of stomach contents into your lungs, which will cause a chemical pneumonia, and potentially lead to death. Now, you have two choices. You can either listen to the surgeon and anesthesiologist, an increase your chance of survival, or you can be incredibly self-centered and stupid, smoke and drink before surgery, and live with the results. Hope that wasn't too blunt, but your question really pisses me off. We give you these instructions for a reason, not torture you. Nobody but you cares whether you have a fistula, and nobody cares but you whether you have this fistula corrected. But if you don't want to do the proper preoperative care, don't bother the surgeons because you can't refrain from smoking or drinking for 12 hours.
