PCOS and Periomenopause- treatment options?

I have had 3 doctors confirm PCOS, the most recent 2 were endos. The first endo recommended spironolactone (I am not trying to conceive, aged 44). The second endo recommended birth control pills, to reduce risk of cancer. I want to try natural hormones, and he agreed to let me work with a compounding pharmacy, maybe using progesterone, etc. He said he expects me to see no results with those type hormones, however, since I am not in menopause. (I need relief from mood swings, irritability, fatigue.) I have had some result using over the counter progesterone creme, ( 4 regular cycles for the first time ever!), and some relief from anxiety, irritablilty). The endo said that he would be amazed if the progesterone could cause that..not strong enough. Thoughts on treatment options would be appreciated.
