nipples...ouch

I am 4 months pregnant with my 3rd baby. After the birth of my last child I left the hospital breastfeeding just fine. About 1 week after release from hospital I started having severe pain while breastfeeding. The pain came from my nipple itself. Went to a lactation consultant and my own Dr. We never found out what the problem was. It lasted about a month. I continued to breastfeed until my baby was 10 months. I have recently (in this past month) again develped a terrible pain in my nipple. It feels as if there is a sandspur or piece of glass in my nipple. If I touch or squeeze my nipples it causes pain. I was leaking colustrum and I am not any longer. I can't imagine what is wrong. I realize I see my doctor every month and will see her shortly but I don't seem to communicate very well with her...as if she just brushes me off. I would like someone else's insight into this. Is there a test that can be done? A culture or something? This is painful to say the least. I want to breastfeed without all the previous complications. My youngest is 16 months old so I didn't stop breastfeeding all that long ago.
