Post menopause symtoms

I abruptly stopped having periods 3 years ago. My blood test showed that I was in menopause. I am 48 years old. I am taking a low dose of hormone replacement because I smoke. My symtoms after menopause include severe acid reflux, extreme muscle stiffness,extreme loss of energy, hot flashes, headaches, and weight gain. Approximately 2 weeks ago my breasts became extremely tender and if I bumped myself it felt as if burning pain went through my milk glands as if my milk were coming in. I also started feeling sick to my stomach and now I've started vaginal bleeding. Does this sound like hormonal imbalance? If so would it be too much hormones or too litle.
