Swelling, itching, prickly labia

I am 37 yr.old, with 15 mo. old twins (C-section, breech and transverse) and I'm 32 weeks pregnant. Both pregnancies normal, usual easily-dealt-with complications (water retention, early delivery) with the twins. For the past 1-2 months I've had a lot of discomfort and irritation around my labia. There has been some swelling, although nothing severe. 2 months ago my OBGYN diagnosed a yeast infection, gave me the usual cream, and the itching and discharge cleared up. Within a couple of weeks, however, the discomfort and swelling returned, although no discharge. The swelling is on only one side of the labia majora, and on the other side of the labia minora. There is a lot of prickly-ness, periodic burning and itchiness, and plain old discomfort. It wakes me up throughout the night as well. I will of course mention this to my OBGYN next visit, but would like feedback from others. I like this dr. a lot, but he pointedly felt my idea of "swelling" was minor compared to what he's seen. I'm a relatively new patient for him (started seeing him at 5 mos. pregnant) so I don't think he knows that for me to lodge physical complaints is highly unusual. The swelling may be small compared to what he's dealt with, but it's very abnormal compared to what my labia usually are! Could this be a Bartholin's cyst? Hernia? Normal body reaction to a quick second pregnancy? (Never got my period back after first pregnancy, stopped nursing when 3 mos. pregnant again.) Thanks if advance for any information.
