When ya really gotta go, your bladder may be distended and full and unable to contract rapidly enough for the "fast stream". Usually not a problem. Try to not get that "full".

Mid cycle spotting is very common and rarely a problem. Without examining a patient, I can't comment further. You can always share this with your doctor and see what s/he says.
