The high levels of HCG in pregnancy frequently cause the ovaries to have many cysts on them at the time of delivery. Why they should rupture, I have no idea.

Depends on many things. One cause of not dilating is the baby being bigger than the pelvis. Another is ineffective uterine contractions.
