Chronic PID is usually pain. Acute exacerbatioins of Chronic PID can be the whole picture - temperature, pain, elevated white blood count, etc. For pure chronic with pain only, antibiotics shouldn't help, surgical removal may. With acute PID, antibiotics are usually used first.
