Dear Lisa,

When you are standing, can you see or feel any bulges at or near the mouth of your vagina? Use a mirror to look.

If your gynecologist has not examined you while you were standing, it is possible that you have an undiagnosed prolapse. Certainly your symptoms, as you describe them, correspond to the symptoms I had several months postpartum - and I was found to have cystocele (bladder prolapsed into the vagina) and rectocele (rectum prolapsed into the vagina), as well as a mild uterine prolapse. Subjectively, it felt like my internal parts were falling out of me.

In any event, it would not be a mistake to see a urogynecologist for a second opinion, and verify at least that the episiotomy/tear should not give you further trouble.
