8 y/o daughter is 'closed'

My 8 y/o daughter's vaginal opening is "closed". (A father's dream!) She was put on estrace cream 2 years ago and still has not had any thinning of this skin. It was mentioned to me by the doc that a surgical procedure may be necessary before she begins to menstruate. She is nearly 9 now. When should I become more concerned and push for more aggressive treatment, such as the surgery?
