The bladder sits very close to the vagina. Sometimes during sex, the bladder can get "bumped" by the penis hitting against the vaginal wall. (Usually when I explain this occurrence, I have a picture nearby. I hope you can envision the anatomy). That bumping can set up an area of irritation in the bladder that's more likely to get infected. It's less likely that a person will get "bumped" if she empties her bladder before sex.

It's also possible that sex, because of the moisture and rubbing, provides an opportunity for urinary tract infection-causing bacteria to get near the urethra (the tube that brings the urine from the bladder to the outside world). For that reason, urinating after intercourse can also help decrease the incidence of sex-associated UTIs, or "honeymoon cystitis."

Sex isn't the only cause of UTIs/bladder infections/urethritis. Women and girls who are not sexually active can also get UTIs.
