abdominal hysterectomy vs LAVH

Thank you in advance for any post concerning my question. After discussion with my gynecologist, I have decided to have a hysterectomy. I have been diagnosed with endometriosis and underwent a diagnostic laparoscopy in 1999 in which the endometrial implants were excised and followed this up with a 6 month course of Lupron therapy. I am now back to 2 weeks of agonizing discomfort each month, accompanied by back pain, nausea, vomiting and diarrhea. Birth control therapy is out of the question because of a diagnosis of pseudotumor cerebri which required the placement of a lumboperitoneal shunt to control my intracranial pressure and preserve my vision. With oral contraceptives, the extra estrogen aggravates this condition. I am aware that the recovery time is shorter and there is less discomfort with the LAVH, but I want to make sure that this shunt tubing does not accidentally become kinked or possibly pulled out through the vaginal incision during this surgery. Would the abdominal approach yield the best results in allowing for better visualization in the operative field. I've had 2 previous C Sections and bilateral tubal ligation. Thanks again for any input.
