MD Challenge...Multiple symptoms -- common cause?From: Girlie (anonymous@obgyn.net)Thu, 21 Dec 2000 04:42:46 -0600 (CST)
I'm a 27 year old Filipina, never pregnant. Diagnosed with migraines since I was 9 yrs old. Rectal polyp removed in 1992. Appendix removed in 1995. Height: 4'11" Weight: 83lbs. Vitals- Avg. Temp: 99 Avg BP: 92/54. Continuing, long term troubles (10 years or more): 1. Constant headache of varying intensity, full migraines twice a week. 2. Frequent vertigo w/vomiting both with and without migrane. (Recent treatment w/Meclizine fairly successful.) 3. Frequent urination (more than twice per hour). 4. Irregular, painful menstruation (normally overdue 1day to 2weeks). (Oligomenorrhea.) 5. Frequent constipation (diet sensitive). Recent troubles (past 2 months): 1. No menstruation for 2.5 months (first time ever). 2. Lower abdominal pain, bloating. Left and right lateral, shooting pain, alternating l-r. Sitting too long makes it worse, standing too long gives "pulling" pain. Unable to work. 3. Constant constipation...very hard stool. 4. Very sensitive breasts. 5. Highly irritable (of course!), and low appetite. Three visits to the clinic in past 2 months, including one to OB. Pregnancy tests negative. 1. GP: Pap smear/swab. Lab test showed non-specific (ovarian?) infection. Antibiotics prescribed. Pain subsided but returned. OB refferal. 2. OB: Gave consult only -- no exam. Suggested abdominal pain due to no ovulation. 3. GP: Pain increasing. Recieved pelvic exam, urinalysis. Suggested Motrin for pain, probably due to ovulation. Questions: 1. Any opinion on a possible common cause for most or all of these symptoms? Endometriosis? Hormonal imbalances? Endocrine/Pituitary disorder? 2. Are my vitals normal for my height and weight, or might they indicate a cause of some of these problems? (Hypotension?) 3. Since my care providers seem reluctant, what specific tests should I request next, and how might I get their attention to "force their hand"? Thanks for the help!
