You say that you touch her through her underwear - this way, there's no telling which part of her is causing that smell. The vagina and the urethra are so close together, the secretions and possibly urine mix easily. There might be a medical problem, but why not take care of the obvious possibilities first: Does she wipe herself after peeing (sounds elementary, but who knows)? How often does she shower? Change her underwear? Are her panties cotton or polyester (which might cause her to sweat more, causing unpleasant smells)? Or maybe she uses too many products for her genital regions, like douching, feminine deodorant etc., which could cause irritation or even infection?

Find a non-threatening way of asking her if she has noticed that smell too, and if all easy solutions don't work, get her to see a doctor.
