Our transcriptionist returned a tape to me today because she could not understand it. Turns out that the battery on the dictaphone was low, but I did not get a warning. If she had not told me this would have been a "lost" dication. Instead, I had to use fast-forward and play at the same time, listen to my tape, and redictate on another machine. What a pain. Dictations can get lost, tapes can break, etc. Your doctor probably takes "chicken scratch" written notes to jog his or her memory, and can often redictate based on these. Best wishes,
