Our transcriptionist returned a tape to me today because she could not understand it. Turns out that the battery on the dictaphone was low, but I did not get a warning. If she had not told me this would have been a "lost" dication. Instead, I had to use fast-forward and play at the same time, listen to my tape, and redictate on another machine. What a pain.

Dictations can get lost, tapes can break, etc. Your doctor probably takes "chicken scratch" written notes to jog his or her memory, and can often redictate based on these.
