Other than discomfort with insertion and removal, you should not be able to feel a pessary once it is in place. You should go back to your doctor tomorrow and have the pessary removed and a different sized pessary inserted. Leaving a painful pessary in for any significant length of time may cause sores or infection in the vagina, and, at worst, erosion of the pessary into the bladder or rectum, leading to a hole (fistula) between these organs and the vagina.
