If the hymen is so thick or large that it has a very small opening, it sometimes needs be opened surgically. If there is a concern that this need be done, some type of anesthesia should be used. It can be quite painful. If you aren't ready to have intercourse, it seems resaonable to wait. You could try to gently stretch it yourself over time - that may make surgery unecessary.
