Is hydrosalpinx the only reason for dialated fellopin tube?

I have had 3 HSG in last 5 years and all passed clear but every time I have pelvic ultrasound, a dialated tube(?) shows up. The last HSG I had earlier this year was to confirm if I have hydrosalpinx and the dye went through Okay. Dr. concluded that we might be seeing enlarged vein and did not do any more check. I am a infertility patient with slightly elevated FSH. I have had 5 IVF faliure despite the fact that I had a good quality eggs. I guess my questions are: - Could one have hydrosalpinx and have good HSG result? - What could cause fellopin tube to be enlarged and is it Okay to have a tube like that? - What I should be doing for the next step? I REALLY need to know what is causing to my infertility....

Thank you,
