You have an excellent grasp of your medical situation and your hormone
status. It's not reasonable, from your history, that these vulvar
effects should be happening from "menopausal changes" and Megace is not
know to have that effect.
It may be time to seek a vulvar specialist at the nearest University
hospital for some real answers to some real quesions.
Unfortunately, vulvar complaints are not diagnosable over the internet
...... yet.
