The only accurate way to diagnose incompetent cervix is to have lost 1 or 2 babies. It usually presents as a history of recurring losses with each one being earlier than the one before. The story is that tha woman notices nothing until the bag of water breaks, then she goes to the hospital and is several cm. dilated and then starts having contractions. There is no other way to tell about incompetent cervix for sure. There have been some studies that suggested that shortening of hte cervix by ultrasound was an early sign. Following this, two studies have been done which showed that if patients with shortened cervices were divided into two groups and one group was treated with a stitch around the cervix and the other got no treatment, the results were the same in both groups.
